I request that telephone number be restored to the form. There are instances where I find communicating with a member by telephone to be the best way to resolve an issue. Phone numbers are sometimes found in the member information available through the ODV section of the web-site, whereas the number is otherwise unlisted. Telephone numbers are present in our records today. Why don't we do our best to keep them up to date. Particularly, if this form eventually updates all relevant records automatically, there will be no human effort required by staff!
