In my opinion (humble or not), this mode of thinking although commendable in general is nothing more than an extension of the huge amount of "hind site says we ought to have been perfect" thinking surrounding Katrina, etc. One does not get hit with such a huge catastrophe as Katrina without suffering ill consequences. Those who feel they must identify systems that "must have failed grossly" to allow the calamity to occur are on target only to the extent that we should have done better. Expecting we or anyone would have managed such a wholesale situation perfectly refuse to accept the imperfection of mankind. We learn mostly through error. Those who commit no errors, do nothing. Those who accept nothing but perfection stymie progress. Those who strive to rescue perfection out of error are the ones who achieve progress.
