On Friday I managed to reach Ian Martinez, Communications Manager at the Telecommunications Industry Association, and asked him for the source of the 200,000 figure cited in the NTIA Broadband Report. He mentioned two research firms that TIA had relied on: In-Stat and Wilkofsky Gruen Associates.
I contacted In-Stat first. Here is their reply:

Hi Dave, The 200,000 number for BPL subs did not come from In-Stat. In our US broadband forecast, we estimate about 231,000 broadband subscribers in the "other" category besides DSL, cable, satellite. Other includes BPL, but is not solely BPL. Hope this helps.
